Typically, the reason is that you’ve forgotten to update the TOC/LOT/LOF – these lists don’t update automatically.
Instead, Word provides a TOC template with placeholder text and you type in each entry manually.

You can also create a custom table of contents with the options you choose and any custom styles that you've applied by using the Table of Contents dialog box. You can type table of contents entries and use tabs to get the dotted lines, or dot leaders, between each entry and its page number.
